Ontario clinics and waitlists, plus attachment help

Explore selected service groups in Aurora, Newmarket, Toronto and the Greater Hamilton Area. This is not a complete clinic list, a distance ranking or a match to your postal prefix. For other communities, use the official provincial and territorial pathways below. A missing local listing does not mean no care is available.

Source check times and notice dates are shown on each card. An advertised intake is not a confirmed opening, appointment or acceptance. Confirm directly before making plans.

  • Provider advertises intake: its page describes a new-patient route. Ask the provider whether you can join.
  • Waitlist: you may ask to join a queue, not book a confirmed appointment.
  • Attachment / navigation route: a service helps you find ongoing care; it is not a verified clinic opening.

Provider advertises intake

Summerview Medical Clinic

Location context: Aurora and Newmarket locations

The clinic advertises new-patient intake at both locations. Use its new-patient or Meet & Greet route, not the existing-patient booking option.

Contact / next step: 905-503-4001: extension 2 for Aurora, extension 1 for Newmarket.

Catchment and detailed eligibility are not verified. Confirm the physician, location and enrolment process. The two locations share one intake notice.

The official intake page offers a registration link for Davis Drive and telephone registration for the Mulock clinic.

Contact / next step: Mulock registration: 289-903-3555. Davis Drive: use Register Online on the linked official page.

Mulock serves local residents without a family doctor or healthcare provider; precise catchment is not verified. Davis eligibility needs confirmation. Do not extend this notice to other Southlake sites.

Access Alliance

Location context: Toronto: College, Danforth and Jane sites

Primary care focuses on immigrants, refugees and newcomers in Toronto, and people facing barriers to care. Interpretation is available. Use the new-patient sign-up route.

Contact / next step: College: 416-324-8677. Danforth: 416-693-8677. Jane: 416-760-8677.

Staff must confirm individual eligibility and placement. Three locations are one organization, not three verified pools of openings. Online appointments for registered patients are separate.

Waitlist

South Riverdale Community Health Centre

Location context: Toronto: South Riverdale / Moss Park

The primary-care page states that new clients can request a registration form and that a waitlist applies. Services include people with and without OHIP.

Contact / next step: Primary-care registration enquiries: 416-461-2493.

Exact enrolment catchment and transfer rules are not verified. Same-day urgent care for registered clients is not a general new-patient opening.

Source checked . Intake notice undated; confirm directly.

Attachment / navigation route

Hamilton Family Health Team — Patient Attachment Team

Location context: Greater Hamilton Area

Register with Health Care Connect, then contact the Patient Attachment Team about finding a family doctor. If already registered with Health Care Connect, use the local contact step. This is a navigation service, not a verified clinic opening.

Contact / next step: Patient Attachment Team: 365-317-8117 or attach@hamiltonfht.ca.

The service is described for people without a family doctor in their area. Confirm the exact catchment and any transfer situation with staff; do not leave existing care merely to join a list. The current source warns that a reply may take 4–6 weeks; that is not a time-to-placement promise. A response or referral does not establish a confirmed opening, appointment or match.

Questions to ask a clinic

Use this checklist yourself; SubeR does not contact clinics or register you.

  • Ask whether they are accepting new patients for ongoing primary care, not just one-time visits.
  • Confirm the service area, eligibility and whether you need a registry referral or can apply directly.
  • Ask how intake works, what happens next, and how to follow up if there is no place now.
  • Ask about accessible visits, language support and a location you can realistically reach.
  • Keep any notes privately. Share health-card or medical information only with the verified official service when required.

If a listing is full or out of date, ask the clinic or official programme about the next appropriate local route. A directory entry is not a promise of care.

Privacy, sources & limits

These choices stay in this open page’s memory. This tool does not save them, request your device location, make provider searches in the background, or send entries to a server. Clear, reload or leaving the page clears the tool’s entries. Copy uses your clipboard only when you press the button.

Official links open another website under its privacy policy. No city or postal prefix is attached to those links. Browsers may retain history, autofill or session recovery. Shared SubeR appearance, bookmarks and weather remain separate; the host still receives normal page and asset requests.

Province and territory pathways reviewed 1 November 2026; local source check times are shown on each card. Original summaries explain navigation, not medical advice. No appointment, matching outcome, complete provider coverage or current availability is guaranteed. This is not an urgent-care service.
